29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. On or about April 15, 2014 and prior thereto, the openings in the guard on the front of the Accurshear were large enough to allow employees fingers under the guard and between the openings in the guard. Employees were exposed to the hazard of the hold downs coming down on their fingers. b. On or about April 15, 2014 and prior thereto, the rotating chuck and drill bit on the JET-13R drill press located outside QC office was not equipped with a guard. Employees were exposed to the hazard of being caught by the rotating chuck and bit. c. On or about April 15, 2014 and prior thereto, the rotating chuck and drill bit on the Ryobi drill press located in the assembly area was not guarded. Employees were exposed to the hazard of being caught by the rotating chuck and bit. d. On or about April 15, 2014 and prior thereto, the rotating chucks and hole saws on the Chas. G. Allen Co. 6 head drill press were not guarded. Employees were exposed to the hazard of being caught by the rotating chucks and saws. e. On or about April 15, 2014 and prior thereto, the back of the Accurshear was not guarded, exposing employee to the hazard of contacting the moving blade or being struck by parts falling out of the back of the shear.

Employees were exposed to the hazard of the blade contacting their fingers.
