29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    On or about March 19, 2015 and prior thereto, employees performing set up and die changes on equipment such as but not limited to the Accurpress press brake,  Premier press brake and Ermak press brake were not deenergizing and locking out the power sources to the presses, exposing employees to the unexpected energization of the equipment.      The Alternative Design Manufacturing and Supply was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(4)(i), which was contained in OSHA inspection number 969830, citation number 1, item number 3 and was affirmed as a final order on September 8, 2014, with respect to a workplace located at 3055 Cheri Whitlock Drive, Siloam Springs, Arkansas.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a.  On or about March 19, 2015 and prior thereto, the guard did not extend completely around the rotating chuck and bit on the JET-13R drill press located outside the QC office.  Employees were exposed to the hazard of being caught by the rotating chuck and bit.    b.  On or about March 19, 2015 and prior thereto, the guard did not extend completely around the rotating chuck and bit on the Ryobi drill press located in the assembly area.  Employees were exposed to the hazard of being caught by the rotating chuck and bit.    c.  On or about March 19, 2015 and prior thereto, the guards did not extend completely around the  rotating chucks and hole saws on the CHAS. G. Allen Co. 6 head drill press.  Employees were exposed to the hazard of being caught by the rotating chucks and saws.
