29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: a) On or about April 24, 2014 - at a jobsite located at 504 E. 43rd Street, Chicago, IL - an employee working in an excavation measuring approximately six (6) feet, five (5) inches in depth and fifteen (15) feet by fifteen (15) feet in length and width installing foundation footers was not protected by cave-in protection, thereby exposing the employee to cave-in hazards.
