A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees. Jobsite: Employee entered a 4 foot 6 inch trench 40 feet long with no ladder access.

29 CFR 1926.601(b)(5): All vehicles with cabs shall be equipped with windshields and powered wipers. Cracked and broken glass shall be replaced. Vehicles operating in areas or under conditions that cause fogging or frosting of the windshields shall be equipped with operable defogging or defrosting devices. Jobsite: Two excavators on site had broken front windshields needing replacement.
