29 CFR 1910.119(e)(3)(i):     The process hazard analysis did not address the hazard of the process:        Worksite located at 4334 S. 67th Street, Omaha, NE; Throughout the facility:  The Process Hazard Analysis (PHA) which was performed between 2013 and 2104 failed to address some of the hazards of the process.  Some of those identified items include the failure of ventilation components, line breaking (piping or valving failure causing a line break), and fire suppression system failure.    Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of the abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citations.

29 CFR 1910.119(e)(5):     The employer did not assure that PHA recommendations were resolved in a timely manner      Worksite located at 4334 S. 67th Street, Omaha, NE; Throughout the facility:      Items identified during the (PHA) which was performed between 2013 and 2104 were not addressed in a timely manner.  Some of those identified items include but are not limited to the installation of vapor monitoring systems (lower explosive limit/upper explosive limit) and a system all-stop.  These two items were identified as corrective actions for a number of hazards identified during the PHA.            Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of the abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citations.

29 CFR 1910.119(j)(2):     The employer did not establish written procedures to maintain the on-going integrity of process equipment:    Worksite located at 4334 S. 67th Street, Omaha, NE; Throughout the facility:  The employer has not developed procedures for ensuring the mechanical integrity of the system.  This would include a program outlining preventative maintenance, component replacement, and/or repair.  One such example would be a pump replacement procedure.        Abatement certification is required for this violation. The abatement certification sheet is enclosed for the citations.
