29 CFR 1926.454(a)(1): The employer failed to have each employee who performs work while on a scaffold trained regarding the nature of any electrical hazards, fall hazards and falling object hazards in the work area. 55 Preston, during roofing work, on or about 5/13/14: Employee working on a ladder jack scaffold platform at the back of the house was exposed to a fall hazard of approximately 12 feet when he did not tie off to the vertical lifeline system while working on the scaffold platform.

(a) 55 Preston, during roofing work, on or about 5/13/14: Employees were exposed to fall hazards of approximately 10 to 12 feet when they were not adequately trained in the installation and use of the vertical lifeline system, including safe use of the rope grab and installation of the MSA roof anchor.
