29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:      a) On or about July 11, 2014 and at times prior thereto at the job site; employees were using a rope grab system with their lanyards for fall protection. The rope grab system was rigged and being used in a way that in the event of a fall the employee would fall greater than 6 feet and possibly contact the ground below.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:      a)  On or about July 11, 2014 and at times prior thereto at the job site; employees were using a ladder to access the garage roof that was extended less than a foot over the landing.      b)  On or about July 11, 2014 and at times prior thereto at the job site; employees were using a ladder to access the main roof that was extended less than a foot over the landing.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):       a) On or about July 11, 2014 and at times prior thereto at the job site; an employee was performing roofing work on a steep roof over 10 feet from the ground to the eave without the use of fall protection.      The BLD Contracting Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 313764425, citation number 1, item number 2 and was affirmed as a final order on 08-08-2011, with respect to a workplace located at 52 Vly Rd, Albany, NY 12205.    The BLD Contracting Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 313759359, citation number 1, item number 2 and was affirmed as a final order on 07-19-2010, with respect to a workplace located at 1060 Western Ave, Albany, NY 12203.
