Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact. On or about May 21, 2014 - at a jobsite located at 1137-39 N. Leavitt Street, Chicago, IL. - employee(s) performing masonry activities was cutting bricks without proper personal protective equipment, including, but not limited to safety glasses and respiratory devices, thereby exposing the employees to eye injuries and health hazards.

Leavitt Street, Chicago, IL. - an employee performing masonry activities from a ladder jack scaffold was using a scaffold plank that was less than 12 inches (30 cm) in width, thereby exposing the employees to fall hazards of approximately eight (8) feet.
