29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold:  a. On or about October 15, 2015, at the above addressed jobsite, an employee performing masonry operations from a pallet on the forks of a powered industrial vehicle (Lull 644E-42), approximately thirteen (13) feet above the ground, was exposed to injuries from falls due to the lack of guardrails.   DREAM WALL ENTERPRISES, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 1926.451(G)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 977862, CITATION NUMBER 1, ITEM NUMBER 3 AND WAS AFFIRMED AS A FINAL ORDER ON SEPTEMBER 23, 2014, WITH RESPECT TO A WORKPLACE LOCATED AT 1137-39 N. LEAVITT STREET, CHICAGO, IL.
