29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification which included the requirements as outlined in 29 CFR 1910.132(d)(2):   a. On May 20, 2014, the employer had not certified a personal protective hazard assessment been conducted which outlined what employees need to wear when performing a specific job task. The workers wore a surgical paper mask, ear protection, safety glasses and shoes while using lead containing ammunition and shooting at a controlled target.

On May 20, 2014, the employer had not conducted an initial determination for employees using and shooting guns and maintaining the indoor shooting range.  A worker was monitored for lead and found exposed over the action level of 30 micrograms per cubic meter. The worker wore only a surgical mask for protection while using ammunition containing lead.
