29 CFR 1910.178(p)(1): Powered industrial truck found to be in need of repair, defective, or in any way unsafe, were not taken out of service until it has been restored to safe operating condition:   a) Shipping and Receiving Dock Areas - The employer failed to ensure that all powered industrial trucks in need of service were removed from service until repairs could be made.  The powered industrial truck that was found to have unsafe conditions was the TCM model #FG25N3T, serial #A23T45008, which had the rubber covers off the foot pedals making them slippery for the operators to operate the forklift.
