Each outlet box in completed installations did not have a cover, faceplate , or fixture canopy:   On or about May 21, 2014 employees were exposed to electrical shock hazards:       a)  In the parts house a junction box was missing a cover.       b)  In the tool pusher trailer an electrical outlet was missing a face plate.
