29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: On or about June 5, 2014, employee was exposed to a fall hazard greater than 6 feet in heght to paved ground level below while working from rooftop without fall protection installing an extension cord connection on a commercial building.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards: On or about June 5, 2014, employee(s) were not trained to recognize fall hazards and were exposed to a fall hazard greater than 6 feet in height to the paved ground below while not wearing fall protection when installing an extension cord on rooftop to gain a power connection to work being performed at ground level.
