The employees were not utilizing a means of fall protection while working and walking approximately 9-25 feet above ground level on the elevated roof, Installing shingles and other roofing materials along the elevated roof, on or about 06/05/14.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access: a. 5470 Lilac Pass, Lot #233, Post Brook Farm Subdivision, Cumming, GA - Employees were exposed to fall hazard. Employees were using portable ladders that did not extend 3 feet above the upper landing while accessing the ladder to get to the roof of the residential home, on or about 06/05/14.

29 CFR 1926.1053(b)(4): Ladders were not used for the purpose for which they were designed: a. 5470 Lilac Pass, Lot #233, Post Brook Farm Subdivision, Cumming, GA - Employees were exposed to fall hazard. Employees took apart a 28ft. aluminum portable extension ladder and use one section of the ladder as a means of access and the other section as a crawling chicken board ladder which was used as steps to gain access to an upper level roof area, on or about 06/05/14.
