29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: (Construction Reference 1926.103)    a) ALO Cinema One Incorporated located in Hackensack New Jersey; On or about June 18, 2014 the employer failed to establish and implement a written respiratory protection program to address identification and evaluation of respiratory hazards, medical evaluations, fit testing, proper use of respirators, maintenance and care of respirators, training and information, program evaluations, and record keeping. Employees were required to wear a negative pressure filtering face piece respirator during drilling operations. Employees were exposed to hazards from silica, including silicosis and lung cancer.

Employees were required to wear a tight-fitting filtering face piece respirator during concrete drilling operations. Employees were exposed to crystalline silica hazards including silicosis and lung cancer.
