Ireland Marine Transportation, Inc., Yard Area:  On or about 06/19/14, employees were using the Dewalt saw that was being supplied power by two extension cords without the use of GFCI protection.

Ireland Marine Transportation, Inc., Yard Area:  On or about 06/19/14, an employee was using the Lincoln Electric welder that had tape covering the damaged insulation 9" from the electrode holder.
