29 CFR 1926.602(c)(1)(ii): Modifications or additions affecting the safe operation of the equipment were  made without the manufacturer's written approval:  On or about June 17, 2014, at the jobsite located at 10915 Eicher Dr., Lenexa, KS:  A contracted employee engaged in electrical installation activities, to include light fixture installation, from a job-made man basket and forklift owned and operated by Horn Distributing was exposed to fall hazards of approximately 20 feet from the elevated forks to the ground below in that the man basket was not approved by the manufacturer.

29 CFR 1926.602(c)(1)(viii)(A): The industrial truck was not equipped with a safety platform/cage firmly secured to the lifting carriage and/or forks when used for lifting personnel:  On or about June 17, 2014, at the jobsite located at 10915 Eicher Dr., Lenexa, KS:  A contracted employee engaged in electrical installation activities, to include light fixture installation, from a job-made man basket and forklift owned and operated by Horn Distribution was exposed to fall hazards of approximately 20 feet from the elevated forks to the ground below in that the man basket was not secured to the forklift mast.
