29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative. Production Area - On or about June 20, 2014, a written energy control program (lockout/tagout) consisting of training, specific procedures and periodic inspections was not available for employees exposed to hazards of moving parts when performing service and maintenance on the Riehle Grouper, Marshall Oven, and other operating equipment.

29 CFR 1910.219(c)(4)(i): Projecting shaft ends shall present a smooth edge and end and shall not project more than one-half the diameter of the shaft unless guarded by nonrotating caps or safety sleeves. Pastry area - On or about June 20, 2014, a projecting shaft end on the Marshall Oven, more than one half in diameter, was not guarded. Employees were thereby exposed to a unguarded rotating shaft.
