Port of Los Angeles, Berth 209, Aboard MV Navios Vector, hatch four on or about June 16, 2014: Two employees discharging cargo in the hatch were exposed to injury when the crane operator was not briefed of his responsibilities to evacuate the employees in the hatch in the event of an emergency.

29 CFR 1918.24(a): There was not at least one safe and accessible ladder for each gang working in a single hatch.  Port of Los Angeles, Berth 209, Aboard MV Navios Vector, hatch four on or about June 16, 2014: Two employees were exposed to injury when at least one accessible ladder was not available for use during an emergency while discharging bulk cargo.  Pasha was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1918.24(a), which was contained in OSHA inspection number 110168, citation number 1, item number 1, and was affirmed as a final order on February 1, 2013 with respect to a workplace located aboard Marine Vessel Glorious Rena. Berth TI 118, Los Angeles, CA 90802.
