29 CFR  1918.24(e)(1): Ladders or steps of adequate strength were not furnished when it was necessary to obtain access to or from a stowed deckload or other cargo, and no other safe means of access were available.  M/V Pan Grace, Hatch 5: On and/or before August 17, 2023, Longshoremen used pieces of wood that were lodged between the bulkhead and metal roll to obtain access to a higher level of the stored cargo. Employee(s) were potentially exposed to fall and/or trip hazard(s).

29 CFR 1918.32(b): The edge of a hatch section, or of stowed cargo, was not guarded by a vertical safety net or other means providing equal fall protection to exposed employees.  M/V Pan Grace, Hatch 5: On and/or before August 17, 2023, longshoremen walked on top of metal cylinders to access a fixed ladder. The metal rolls were not guarded by a vertical safety net or other means providing equal fall protection. This condition possibly exposed employee(s) to fall hazards.
