29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  a.   On or about June 25, 2014, employees installing OSB board roof sheathing on the 5:12 and 7:12 pitch roofs were exposed to 11 feet to 19 feet fall hazards, in that they were not protected by a fall protection system.  b.   On or about June 25, 2014, a guardrail system was not installed on the stairs to the second floor, exposing employees to an 11 feet fall hazard.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who was exposed to fall hazards that enabled each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards:  a.   On or about June 25, 2013, employees working from 5:12 and 7:12 pitch roofs at elevations of 11 feet to 19 feet were not trained to recognize fall hazards and install the fall protection equipment provided by the employer to minimize these fall hazards.
