29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:  a. The employer fails to ensure effective protection against falls as required by the standard, as demonstrated on February 5, 2018, on the 8:12 pitch roof, an employee installing decking was not protected from a 24 feet fall hazard by the use of a fall protection system.   Ezzo Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1088539, citation number 1, item number 2 and was affirmed as a final order on March 11, 2016, with respect to a workplace located at 9501 Abby Glen Drive, Jacksonville, Florida 32099.  Ezzo Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 982360, citation number 1, item number 1 and was affirmed as a final order on September 5, 2014, with respect to a workplace located at 14950 Venosa Cir, Jacksonville, Florida 32258.
