29 CFR 1910.215(a)(4):  Work rest(s) on grinding machines were not adjusted closely to the wheel with a maximum opening of one eighth inch:    On or about June 26, 2014, employees were exposed to struck-by hazards in that the employer did not ensure that the work rest on the bench grinders were adjusted to the wheel with a maximum opening of one eight inch.  a)  DeWalt 6" bench grinder, 3450 RPM, 120 V AC, 60 HZ, Serial #200518-YL0650, Type 1.    b)  Ryobi 8" bench grinder, BGH827, 120 V AC, 60 HZ, Serial #FG133448985.    The Amarillo Custom Box Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.215(a)(4), which was contained in OSHA inspection number 911293, citation number 1, item number3/a and was affirmed as a final order on 12-11-2013, with respect to a workplace located at 1501 S. Johnson, Amarillo, TX 79101.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guar peripheral member at the top exceeded on fourth inch:  On or about June 26, 2014, employees were exposed to struck-by hazards in that the employer did not ensure the bench grinders tongue guard were adjustment to the wheel did not exceed one fourth inch. a)  DeWalt 6" bench grinder, 3450 RPM, 120 V AC, 60 HZ, Serial #200518-YL0650, Type 1.  b)  Ryobi 8" bench grinder, BGH827, 120 V AC, 60 HZ, Serial #FG133448985.
