On or about June 23, 2014, the operator of the Schwing pump truck, model S41SX, license plate no. B498182, was operating a pump truck boom within 10 feet of high voltage power lines.

29 CFR 1926.600(a)(6)(iv): A person was not designated to observe clearance of the equipment and give timely warning for all operations where it is difficult for the operator to maintain the desired clearance by visual means: On or about June 23, 2014, the employer did not provide to the concrete pump truck operator a spotter to observe clearance of the boom to the high voltage power lines located above the retaining wall form.
