1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  a) On or about March 31, 2022, at 21 Provident Oaks, Bossier City LA, 71111, the employer failed to ensure that employees were adequately trained in the recognition of hazards related to the improper set-up of a pump truck and outriggers during a concrete pour, exposing employees to the tipping over and/or collapse of the pumping truck.
