29 CFR 1926.451(c)(2)(iv):  Front-end loaders and similar pieces of equipment shall not be used to support scaffold platforms unless they have been specifically designed by the manufacturer for such use.      (a) Kruse Builders, LLC at 840 Twilight Ln, Steamboat Springs, CO 80478:  On and before June 19th, 2014 the employer used a Terex all-terrain forklift, model number SS636, to elevate an employee.  While installing roof sheeting, the employee stood on and worked from plywood sheeting that was stacked on the forks of the forklift.  This condition exposed the employee to a fall hazard of approximately 17 feet.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, personal fall arrest system, or other approved alternative fall protection system:      (a)Kruse Builders, LLC at 840 Twilight Ln, Steamboat Springs, CO 80478:  On and before June 19th, 2014 the exposing employer did not ensure that an employee was protected from injury, in that, an employee working near the edge on top of the first level was not utilizing a fall protection system. This condition exposed the employee to a fall hazard of approximately 11 feet.
