Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire and/or explosion hazards:   (a) On or about July 8, 2014, and at times prior to this date, for employees exposed to fire and/or explosion hazard while working from the rod basket during tripping operations where no means of auxiliary escape had been installed for emergency egress at Farms 1, NWNE 2-162-91, Bowbells, ND.      Abatement Note: Among other methods, one feasible and acceptable abatement method to correct the hazard would be for the employer to install a Geronimo emergency escape device in accordance with the Manufacturer's Operators Manual, pages 9 - 17, INSTALLATION.  Also (API) American Petroleum Institute Recommended Practice 54, Third Edition (Recommended Practice for Occupational Safety for Oil and Gas Well Drilling and Servicing Operations), section 6.10, AUXILIARY ESCAPE, subsection 6.10.1, "On all land rigs, the derrick or mast shall have an auxiliary means of escape installed prior to personnel working in the derrick."
