29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed:    (a) Loading Dock - Sam's Club, Silver Spring Commons, Mechanicsburg Pennsylvania - Employees used a six foot step ladder in the folded position to climb from the loading dock apron into the building at the loading dock door and were subject to a fall to the ground on or about July 7, 2014.     No abatement certification or documentation required.    CHANGED TO PER ISA    29 CFR 1926.1051(a):   A stairway or ladder was not provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runaway, sloped embankment, or personnel hoist was provided:    (a)	Loading Dock - Sam's Club, Silver Spring Commons, Mechanicsburg Pennsylvania - Employees used a six foot step ladder in the folded position to climb from the loading dock apron into the building at the loading dock door and were subject to a fall to the ground on or about July 7, 2014.  No acceptable ladder or stair was provided.
