29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  (a)   Fabrication shop area: On or about 07/09/2014, the employer failed to mount several metal receptacles into a fixed position, causing the receptacles to hang from the live wires connecting to the metal receptacle boxes. Employees were potentially exposed to contact with electrical current if any of the live wires became exposed from the receptacle box.

29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposes an employee to injury and the defective or damaged item was not removed from service:  (a) The employer exposed employees to a potential contact with electrical hazard when an extension cord, used to power a table saw in the wood cutting area of the fabrication shop, was defective but still in use after being covered with electrical tape.
