OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to explosion and burn hazards:    (a)  On or about July 16, 2014 - Main plant, an implosion occurred in the cupola used for the manufacturing of rock wool when water circulation in the cooling jacket decreased causing the furnace to overheat and implode.    Among other methods, one feasible and acceptable means of abatement is to install flow and temperature sensing equipment which would detect cooling water circulation problems and automatically shut down the furnace.
