29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    4302 Roosevelt Blvd, Philadelphia, PA - An employee was working on a steep (10 - 12 pitch) roof approximately 10 feet above the next level without the use of a fall protection system. This condition was observed on or about July 16, 2014.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who was exposed to fall hazards that enabled each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards.  a) 4302 Roosevelt Blvd, Philadelphia, PA - Employees were not instructed in the recognition and avoidance of fall hazards and the regulations of their work environment to control or eliminate hazards. The employees were exposed to fall hazards greater than 6 feet. This violation was observed on or about July 16, 2014.
