29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms. a. On or about 07/17/2014, at the job site - Guardrails were not installed for employees installing tyvex wall wrapping paper from a wood platform positioned with one side on the scaffold and the other side on a roof slope was 8.5:12, exposing the worker to an approximate 17 foot fall hazard.
