29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:  On or about July 22, 2014, the employer failed to provide the information in Appendix D to 29 CFR 1910.134 to employees who voluntarily wear dust masks while performing abrasive blasting.    Bescast Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 315312900, citation number 1, item number 1 and was affirmed as a final order on June 2, 2011, with respect to a workplace located at 4600 East 355th Street, Willoughby, Ohio 44094.

Bescast Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 315312900, citation number 1, item number 2 and was affirmed as a final order on June 2, 2011, with respect to a workplace located at 4600 East 355th Street, Willoughby, Ohio 44094.
