29 CFR 1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by, ingoing nip points, rotating parts, flying chips and sparks:  a) 551 Old Swede Road, Douglassville, PA 19518, on or about November 21, 2025 - Employees were exposed to unguarded pinch points on a custom manufactured machine, locally know as the "Knock-out", which is comprised of a Toku TB-90, Air Concrete Breaker and a lifting table.  The lifting table, and its pedestal, were not guarded to prevent the operator from having any part of the body in the danger zone while placing parts into position on the pedestal and while unloading the machine.  Abatement certification required within 15 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.212(a)(3)(ii):  Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  a) 551 Old Swede Road, Douglassville, PA 19518, on or about November 21, 2025 - Employees were exposed to unguarded pinch points on a custom manufactured machine, locally know as the "Knock-out", which is comprised of a Toku TB-90, Air Concrete Breaker and a lifting table.  The Toku TB-90, Air Concrete Breaker, and its chisel, were not guarded to prevent the operator from having any part of the body in the point of operation while parts were being placed into position on the lifting table's pedestal, during chisel operations, and while unloading the machine.  Abatement certification required within 15 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1904.29(b)(1):  The employer did not enter information about the business at the top of the OSHA 300 Log, a one or two line description for each recordable injury or illness, and a summary of this information on the OSHA 300-A at the end of the year:  a) 551 Old Swede Road, Douglassville, PA 19518, on or about January 15, 2026 - Column F for Case dated 11/21 on the 2025 OSHA Form 300 Injury and Illness Log provided the injury and part of the body affected, but did not include the object/substance that directly injured the person.  b) 551 Old Swede Road, Douglassville, PA 19518, on or about January 15, 2026 - the 2025 OSHA Form 300 Injury and Illness Log did not include the establishment's name.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
