29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toe boards on all open sides except where there was an entrance to a ramp, stairway, or fixed ladder.    On or about 29 July 2014, and at times prior thereto, employees accessed an upper level storage platform that did not have railing with a mid-rail or toe boards, exposing employees on the platform to a fall hazard of approximately 9 feet to the asphalt below and employees working underneath to the hazard of being struck by falling materials or tools.  1) Bay 1 had upper level storage in which a single chain was used as the top rail only, and there were no toe boards.  2) Bay 4 had upper level storage in which the railing consisted of a wooden top rail only, and there were no toe boards.  3) Bay 5 had upper level storage in which the railing consisted of a wooden top rail only, and there were no toe boards.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries.      On or about 29 July 2014, and at times prior thereto, fire extinguishers located within the yard area were not mounted or identified so that they could be readily accessed, exposing the employees in the yard to a fire hazard. Employees are expected to use the extinguishers for small incipient fires.   1) At the north end of Shed 2, the extinguisher was not mounted or identified by signage.  2) At the south end of Shed 1, the extinguisher was not mounted or identified by signage.  3) Shed 3 had three extinguishers that were missing signage.  4) At the north end of Shed 1, the extinguisher was not identified by signage.  5) At the yard entrance, the extinguisher was not identified by signage.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling.     On or about 29 July 2014, and at times prior thereto, the employer provided power for three aisle light sets by connecting extension cords in series, or daisy chaining, exposing the employees working in the aisle and area to both electrical and fire hazards.

29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings were not provided with covers identified for the purpose. In completed installations, each outlet box did not have a cover, faceplate, or fixture canopy.    On or about 29 July 2014, and at times prior thereto, an energized electrical outlet that was approximately 7.5 feet in height was missing the right two(2) plugs of a four (4) plug outlet box. Additionally, just below was a pull box that was not effectively closed with a cover. Within the same outlet box was a snap switch used to control the power tool room lights at least twice daily. The open wiring and terminals exposed the employees accessing the snap switch to an electrical hazard of approximately 120 volts.

29 CFR 1910.303(g)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures.  On or about 29 July 2014, and at times prior thereto, an energized open electrical conduit had wire ends with screw caps on them within the tool room which was less than 8 feet in height, exposing employees to an electrical hazard of approximately 120 volts.
