29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews:   Southwest corner of roof, On or about July 30, 2014, employees were exposed to fall hazards greater than 10 feet in height to unpaved ground level below when working from a scaffold that did not have guardrails installed at open end of the extension bracket, while laying block on an exterior wall.

29 CFR 1926.451(g)(4)(iv): Midrails were not installed at a height approximately midway between the top edge of the guardrail system and the platform surface:     Southwest corner of roof, On or about July 30, 2014, employees were exposed to falls greater than 10 feet in height, to the ground level below, when working from a welded tubular scaffold, which was not provided with a midrail, while mixing mortar and handing cement blocks to masons.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.   Southwest corner of roof, On or about July 30, 2014, employees were exposed to a fall hazard approximately 25 feet to the paved ground below when mixing mortar from a scaffold that did have guardrails installed at ends of the scaffold.

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement:  Southwest corner of roof, On or about, July 30, 2014, an employee was exposed to an impalement hazard while mixing mortar from a scaffold platform that was missing a midrail and was erected 19 inches away from 5 uncapped rebar ends.
