29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or (c).  a) West side of residence, 406 Willow Street, Dunmore, PA: Employees were within a trench without an adequate protective system that ranged in depth from five-foot, three-inches (5'3") deep to eleven-foot (11') deep. Employees were exposed to a caught-in hazard, as observed on or about August 7, 2014.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.
