29 CFR 1910.1030(c)(1)(i): The employer having employee(s) with occupational exposure did not establish a written Exposure Control Plan designed to eliminate or minimize employee exposure: Rockdale Newton Medical Practice - on or about August 12, 2014, where employees were exposed to needlestick and bloodborne pathogen hazards when drawing blood from patients, the employer did not develop and implement a written Exposure Control Plan. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.1030(f)(1)(i): The employer did not make available the hepatitis B vaccine and vaccination series to all employees who have occupational exposure: Rockdale Newton Medical Practice - on or about August 12, 2014, where employees were exposed to needlestick and bloodborne pathogen hazards when drawing blood from patients, the employer did not offer the hepatitis B vaccine.
