29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):  Restaurant Entrance - The main 240v, 3-phase electrical panel did not have a dead front trim cover guarding against exposed live parts.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  Restaurant Roof - The employee was replacing shingles on the upper roof while standing on the lower roof approximately 6'4" from the wooden box below without any fall protection.  Wharf on Johnson's Bay was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 901078, Citation 1, Item 2 and was affirmed as a final order on April 30, 2013, with respect to a workplace located at 69 Johnson Street, Lubec, ME 04652.
