Mexamerica Foods LLC; upstairs storage - On or about 8/20/14, a wooden guardrail system was missing a mid rail at the end section.   The fall height was approximately 10 feet to the ground below.

Mexamerica Foods LLC; end of conveyor - On or about 8/20/14, a household fan was being utilized to cool tortilla dough.  The fan is not listed and/or labeled for the industrial environment.     B.)  Mexamerica Foods LLC; freezer wall - On or about 8/20/14,  A section of MC cable was not secured.   MC cable went through wall and connected to a receptacle outlet box.     C.)  Mexamerica Foods LLC; upstairs storage/back wall - On or about 8/20/14, Romex cable was not secured to the wall in accordance with its listing and labeling.  It must be secured every 3 feet and within 12 inches of the receptacle box.    D.)  Mexamerica Foods LLC; packaging line - On or about 8/20/14,  knockout type outlet boxes were hanging below the packing line.  The boxes are designed to be surface mounted and free from physical damage/moisture accumulation.  These boxes are manufactured to be used as a mounted box.
