29 CFR 1910.178(l)(4)(ii)(B): Refresher training in relevant topics were not provided to the operator when: The operator has been involved in an accident or near-miss:    a) Hinkle Manufacturing, Inc., Perrysburg, Ohio:  On or about August 25, 2014, the employer did not ensure that a lift truck operator who had been involved in a lift truck accident received refresher training.  On August 8, 2014, the operator struck a pedestrian while he traveled forward in his lift truck with an obstructed view.

29 CFR 1910.178(n)(4): Industrial truck driver(s) were not required to travel with the load trailing whenever the load obstructed forward view:    a) Hinkle Manufacturing, Inc., Perrysburg, Ohio:  On or about August 8, 2014, the employer did not ensure that lift truck operators who were carrying a load that obstructed their vision traveled with the load trailing.  An employee was injured after being struck by a lift truck that was traveling with an obstructed view down an aisle.
