29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:    a. On September 29, 2014, the employer did not ensure that employees who set-up the Niwa slitter (s/n SC-3000-2) machine were trained in the skills required for the safe application, usage, and removal of the energy controls.  Employees were not trained to follow energy isolating steps to when setting up the machine's blades.

Hinkle Manufacturing, Inc. - Perrysburg, Ohio: On September 29, 2014, lockout devices were not affixed to the energy isolating device for the Niwa slitter (s/n SC-3000-2) before operators setup the machine.  Employees were exposed to rotating parts/blades in the event the machine unexpectedly startup during the setup activities.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    a. Hinkle Manufacturing, Inc. - Perrysburg, Ohio: On September 29, 2014, employees operating the Niwa slitter (s/n SC-3000-2) were exposed to laceration and amputation injury in that the machine's rotating blades and parts were not guarded.     Hinkle Manufacturing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 905407, citation number 1, item number 1 and was affirmed as a final order on September 19, 2013, with respect to a workplace located at 5th & D Streets in Perrysburg, Ohio.
