29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use:   On or about September 3, 2014 at a job site located at State Road, Cuyahoga Falls, OH 44223, during construction activities including but not limited to removing excess mortar from a block wall, the employer did not ensure the JLG G9-43A rough terrain forklift was capable of supporting the platform exposing employees to fall hazards approximately 30 feet.

29 CFR 1926.451(c)(2)(v): The employer used forklifts  to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied:   On or about September 3, 2014 at a job site located at State Road, Cuyahoga Falls, OH 44223, during construction activities including but not limited to removing excess mortar from a block wall, the employer did not ensure the JLG G9-43A rough terrain forklift was not moved horizontally while employees were working in the platform exposing employees to fall hazards approximately 30 feet.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:   On or about September 3, 2014 at a job site located at State Road, Cuyahoga Falls, OH 44223, during construction activities including but not limited to removing excess mortar from a block wall, the employer did not ensure employees working from an elevated platform were protected from fall hazards approximately 30 feet by a fall protection system/device.  LONNIE CROWELL MASONRY INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, 29 CFR 1926.451(G)(1)(i), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 313497265, CITATION NUMBER 01, ITEM NUMBER 001, AND WAS AFFIRMED AS A FINAL ORDER ON MAY 24, 2010 WITH RESPECT TO A WORK PLACE LOCATED AT GLORY ROAD DR, EL PASO, TX, 79902.
