29 CFR 1910.217(e)(1)(i)(A): Periodic and regular inspections of each power press were not conducted to ensure that all of its parts, auxiliary equipment, and safeguards, including clutch/brake mechanism, antirepeat feature, and single-stroke mechanism, are in safe operating condition and adjustment: a) Forming Department - At the time of the amputation incident on or about July 14, 2014 and times prior to, the employer did not ensure that all of the safeguards on Press #206 including but not limited to the brake monitor and the anti-repeat feature were in a safe operating condition and adjustment by having them inspected regularly, thus exposing employees to amputation hazards at the point of operation.
