29 CFR 1910.303(f)(2): Services, feeders, and branch circuits. Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, shall be legibly marked to indicate its purpose, unless located and arranged so the purpose is evident.   a. At the workplace in the Main Production area, circuit breaker panel "D", a 120/240 volt circuit breaker panel, was not legibly marked so as to indicate the purpose of each overcurrent device.  Employees were exposed to electrical hazards while turning lights on and off from the panel.

29 CFR 1910.304(g)(5):   Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent, continuous, and effective.  a. At the workplace in the production area, employees were exposed to an ungrounded 120 volt industrial fan.  The fan's grounding pin was removed/missing causing the fans' body to carry current.

29 CFR 1910.305(b)(1)(ii): Unused openings in cabinets, boxes, and fittings shall be effectively closed.   a. At the workplace in the Main Production area, circuit breaker panel "D", a 120/240 volt circuit breaker panel, had a knockout hole on it's left front facing side that was not sealed.   b. At the workplace in the Main Production area, circuit breaker panel "D", a 120/240 volt circuit breaker panel, had a breaker missing on it's right side branch.
