29 CFR 1910.1200(e)(1): The employer did not maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: (Construction Reference: 1926.59).  (a) On or about September 9, 2014, at the VA Medical Center located at 1030 Jefferson Ave, Memphis, TN, Dunbar Mechanical employees  were exposed to compressed gasses (including but not limited to oxygen and acetylene) and did not have a copy of their written hazard communication program on site.  Without proper information on hazardous materials, employees could mis-handle hazardous materials and expose themselves to skin, eye, and respiratory irritations.  Dunbar Mechanical was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 1910.1200(e)(1), which was contained in OSHA inspection number 905519, Citation Number 1, Item Number 1, issued on 06/18/2013, with respect to a workplace located at 1030 Jefferson Ave, Memphis, TN 38104.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.1200(e)(1)(i): The employer did not compile a list of the hazardous chemicals known to be present using a product identifier that was referenced on the appropriate safety data sheet.  (a) On or about September 9, 2014, at the VA Medical Center located at 1030 Jefferson Ave, Memphis, TN, Dunbar Mechanical employees  were exposed to compressed gasses (including but not limited to oxygen and acetylene) and did not have a chemical list including these materials on site.  Without proper information on hazardous materials, employees could mis-handle hazardous materials and expose themselves to skin, eye, and respiratory irritations.  Dunbar Mechanical was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 1910.1200(e)(1)(I), which was contained in OSHA inspection number 905519, Citation Number 1, Item Number 2, issued on 06/18/2013, with respect to a workplace located at 1030 Jefferson Ave, Memphis, TN 38104.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
