29 CFR 1926.102(a)(1):   Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents.  On 9/11/2014, workers from Juan Martinez were repairing a roof of a residential building located at 22, 24 and 26 Tournament Tee Drive, OFallon MO.   Two employees were not wearing eye protection while using pneumatic nailers and hammers.

29 CFR 1926.501(b)(13):    "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system.  On 9/11/2014, workers from Juan Martinez were repairing a roof of a residential building located at 22, 24 and 26 Tournament Tee Drive, OFallon MO.  They were not utilizing guardrail systems, a safety net system, or personal fall arrest systems.  The owner, Juan Martinez, and four employees were observed on the roof of the building without fall protection, exposing him to fall to the ground below.

29 CFR 1926.1053(b)(1):   When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access.  On 9/11/2014, workers from Juan Martinez were repairing a roof of a residential building located at 22, 24 and 26 Tournament Tee Drive, OFallon MO.  They were using a ladder improperly to access the roof.  The ladder was not extended to three feet beyond the landing or edge of the roof.

On 9/11/2014, workers from Juan Martinez were repairing a roof of a residential building located at 22, 24 and 26 Tournament Tee Drive, OFallon MO.  Employees were carrying bundles of shingles and supplies up a ladder, which could have caused them to lose balance and fall.
