29 CFR 1926.501(b)(13): When another provision in 29 CFR 1926.501(b) did not provide for an alternative fall protection measure, each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:  The employer does not ensure that employees engaged in residential construction activities are protected from fall hazards. This violation occurred on or about September 20, 2014, at a workplace located at 9730 Whitehurst Drive, Dallas, Texas; where:  a) At least two employees, who were on the roof painting the exterior of the building, were not protected from falling approximately 18-feet from the roof's edge to the lower level.  b) At least two employees, who were on the roof painting the exterior of the building, were not protected from falling approximately 20-feet through skylights.

29 CFR 1904.39(a): Within eight (8) hours after the death of an employee from a work-related incident, the employer did not orally report the fatality by telephone or in person to the Area Office of the Occupational Safety and Health Administration (OSHA), that was nearest to the site of the incident, or call the OSHA toll-free central telephone number, 1-800-321-OSHA (1-800-321-6742):  Following the work-related death of an employee that occurred on or about September 26, 2014, at a workplace located at 9730 Whitehurst Drive, Dallas, Texas 75243; the employer did not make an oral report to OSHA within eight (8) hours.
