This violation occurred on or about September 20, 2014, at a workplace located at 9730 Whitehurst Drive, Dallas, Texas; where:  a) At least two employees, who were on the roof painting the exterior of the building, were not protected from falling approximately 18-feet from the roof's edge to the lower level.  b) At least two employees, who were on the roof painting the exterior of the building, were not protected from falling approximately 20-feet through skylights.
