29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):   On or about 1 October, 2014, an employee was exposed to falls approximately 15 feet in height to the unpaved ground level below from a scaffold, which had only 1 out of 5 possible planks installed while employee was installing felt paper on an exterior wall.

29 CFR 1926.451(c)(1)(i): Guys, ties, and braces were not installed at locations where horixontal members support both inner and outer legs:  On or about 1 October, 2014, employees were exposed to a fall hazard of approximately 15 feet in height to ground level below while working from the 2nd tier of the scaffold that was erected without braces being installed at any point of the inner legs of the scaffold.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  On or about 1 October, 2014, employees were exposed to fall hazards approximately 15 feet in height to unpaved ground level below while accessing the 2nd level of a scaffold that had a ladder installed that did not extend at least 3 feet above the scaffold landing for access.
