29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) Roof, new residential construction located at 4508 Sterling Pointe Drive, Kennesaw, GA 30152: Employee was exposed to fall hazards of approximately 18-feet, 5-inches to the lower level while carrying CertainTeed CT-20 Shingles from the ladder to the ridge on a 8/12 pitched roof with no fall protection system in place.

29 CFR 1926.1053(b)(20): Employees did not face the ladder when ascending or descending the ladder:  (a) Front side, new residential construction located at 4508 Sterling Pointe Drive, Kennesaw, GA 30152: Roofers were exposed to slip and fall hazards while not facing the ladder when descending from the roof at eave heights of approximately 18-feet, 5-inches.

29 CFR 1926.1053(b)(22): Employees using ladders were carrying objects or loads that could cause them to lose balance and fall:  (a) Front side, new residential construction located at 4508 Sterling Pointe Drive, Kennesaw, GA 30152: Employees were exposed to falls of up to 18-feet, 5-inches while carrying CertainTeed CT-20 roofing shingles up a Werner (Modell D1124-2) Aluminum Extension Ladder. Employees carrying the 80 lb. bundles could cause loss of balance and falls while needing one hand to hold onto the shingles when climbing the ladder to the eave.
